sturmey-archer made internal hubs, which gave the rider between three and five gears in an internal hub, from 1903 until fairly recently. (http://www.sheldonbrown.com/sturmey-archer.html) Rohloff makes 14 (I think) speed internal hubs now. They just might cost more than your bike.
